The Center for Healthy Aging encourages and assists community-based organizations serving older adults to develop and implement evidence-based health promotion/ disease prevention programs. Evidence-based programming translates tested program models or interventions into practical, effective community programs that can provide proven health benefits to participants.

 

The Administration on Aging (AoA) currently funds two evidence-based disease prevention national demonstration programs.  The first provides grants to community-based organizations for the replication of evidence based, chronic disease self management interventions.  The most recent initiative provides grants to states to promote state-wide dissemination of evidence based health promotion programs.
